I understand that in the event medical intervention is needed, every attempt will be made to contact immediately the persons listed on this form. In the event I (we) cannot be reached, I give my permission for medical treatment to the physician or dentist selected by the adult leaders of the New McKendree United Methodist Church. I understand that my insurance converage will be used as primary coverage in the event a medical emergency occurs. I understand all reasonable safety precautions will be taken at all times by the church and its agents during the events and ativities. I agree not to hold the church leaders, employees, and volunteer staff liable for damages, losses, diseases, or injuries incurred. For promotional and publicity purposes, I will allow my youth's picture to be taken and used. Names will not be used to identify persons in the pictures.

Description

Fields of Faith, a community youth night of worship will be Wednesday, October 15, 7:30 PM, at SEMO's Houck Stadium in Cape. We will meet at the stadium at 7:15 PM and gather by the front entrance to walk in together. Pick up will be around 9:00 PM (sometimes it runs longer than expected) and we will bring students to the same spot for pick up as drop off. If your child needs to be picked up earlier please contact Tylor Barnes at tbarnes@newmckendree.org or 573-625-2785 and we will bring them to the front entrance. Parents/Guardians are expected to meet at the front of the stadium for pick up. No money is needed.
